Ideal Complex Care is a leading provider of complex care, offering support to people living in their own homes in the community, some independently and some with their family.
We are looking to recruit a Nurse Supervisor to case manage our services across primarily East and West Sussex. Reporting to the Senior Branch Nurse, the successful applicant will be responsible for:
- Case management for complex care clients including care planning, risk management and reviews
- Support and supervision of Healthcare Assistants, including competency assessment
- Reporting and records management
- Participating in discharge meetings, care reviews and liaison with health professionals
- Participating in on-call escalation rota.
Key criteria for the role will include:
- An active PIN
- Understanding of community-based nursing / case management
- Strong clinical skills
- Disciplined with a strong work ethos
- Team player
- Full, current driving licence and use of own car
- Must be based in East Sussex and be prepared to travel extensively in the area
