We have a new opportunity for a Permanent Band 6 Children’s Nurse Case Manager in South London.
Key Details:
- Start Date: ASAP
- Shift Days: Monday to Friday
- Shift Hours: 09:00am – 17:00pm, with one evening per fortnight on call. Full flexibility considered.
- Salary: Up to £45,000 per annum depending on experience, plus Oyster Card and mileage reimbursement.
Role Responsibilities:
As a Nurse Case Manager (Tracheostomy Experienced), you will:
- Provide face-to-face support to clients and their families.
- Create personalized care plans for each client.
- Work closely with a core team of carers, training and supporting them with clinical skills and activities.
Client Conditions:
This role involves working with clients with conditions such as:
- Spinal cord injuries
- Neuromuscular disabilities
- Tracheostomy / ventilation
- Acquired brain injuries
- Learning & physical disabilities
- Epilepsy
Candidate Requirements:
- Children’s community nursing experience, RSCN or RN Child Branch Diploma/Degree.
- Evidence of relevant post-qualification experience and ongoing clinical training.
- Experience caring for children with complex health needs, including tracheostomy care and long-term ventilation (oxygen therapy, suction, enteral feeds, catheterisation).
- Knowledge of spinal injury care is desirable.
- Enteral feeding experience is essential.
- A qualified nurse will always be available for advice and support via on-call.
- Experience caring for children with life-limiting or threatening illnesses.
