Job Title: Nurse Case Manager (Tracheostomy Experienced)

Location: North London & South London (Hybrid – office, home and community-based)

Salary: Up to £45,000 per annum (DOE) + Oyster Card + mileage reimbursement

Contract Type: Permanent

Hours: Monday to Friday, 9am–5pm (Full flexibility considered); one evening per fortnight as part of the on-call rota

Overview:

We are seeking 5x experienced and compassionate Nurse Case Managers to join a growing healthcare team, supporting clients across North and South London . This is a hybrid role combining office work, home-based duties, and community field visits.

As a Nurse Case Manager, you will be the primary clinical lead for clients with complex care needs, working face-to-face with clients and their families to develop personalised care plans. You will also play a crucial role in leading, training, and supporting a dedicated team of carers to deliver high-quality care.

Client Conditions Include:

  • Spinal cord injuries
  • Neuromuscular disabilities
  • Tracheostomy and/or ventilated care
  • Acquired brain injuries
  • Learning and physical disabilities
  • Epilepsy

Experience & Requirements:

  • Proven experience in complex care and clinical support
  • Tracheostomy care and/or long-term ventilation experience essential
  • Experience with oxygen therapy, suction, enteral feeding, catheterisation
  • Experience supporting clients with spinal injuries is desirable
  • Must hold relevant nursing qualifications and a commitment to ongoing professional development
  • Enteral feeding experience essential
  • A qualified nurse is always available for clinical advice via an on-call system
