Main Responsibilities
- Support patient care in an in‑patient environment, working within the discharge and rehabilitation pathways.
- Demonstrate flexibility and initiative with caseload, mindful of wider team needs and workload.
- Motivate and encourage patients with physical needs to safely discharge to their previous place of residence.
- Work independently, demonstrating ability to manage admin duties; basic IT skills desirable.
- Assist with moving and handling of adults; recent experience in an inpatient care setting essential.

Person Specification
Education
- Essential: NVQ Level III or equivalent qualification in Health & Social Care.
- Desirable: Evidence of relevant course attended.
Experience
- Essential: Previous experience of working as an Occupational Therapy or Physiotherapy assistant. Evidence of working with rehab patients – inpatients. Evidence of working independently. Evidence of working in an NHS hospital (acute or community) setting.
- Desirable: Previous experience as an Assistant Practitioner role. Evidence of working across a 7‑day service.
Communication
- Essential: Effective oral and written communication skills. Evidence of communicating difficult decisions with patients and families.
