IC24 is seeking an experienced Practice Nurse to join its Primary Care team at Herstmonceux Integrative Health Centre. You will lead diabetes and respiratory care, perform evidence‑based reviews, and educate patients for self‑management within a supportive multidisciplinary team.
You’ll deliver NHS Health Checks, immunisations and travel health guidance, and work with GPs and nurses to provide coordinated, patient‑centred care across the community.
