As a Clinical Navigator GP, you will support the provision of integrated urgent care across IC24’s urgent care portfolio, providing on-shift senior clinical oversight to all IC24 services. This includes Clinical Assessment Services (CAS), Urgent Treatment Centres, Acute Home Visiting, Face to Face Appointments and other linked services across IC24 operational sites. This role is about fostering and overseeing effective communication between our operational and clinical teams, clinical management and applying key principles of safe, effective, resourceful patient care.

You will co-ordinate the use of valuable NHS resources across the different counties covered by IC24 with the aid of a dedicated team of administrative staff. In times of clinical and system stress, you will co-ordinate clinicians’ roles on CAS, patient dispositions, and the patient queues. As the senior clinician on shift, queries concerning time critical or challenging patients will come to you from the navigation team, GPs, ANPs, paramedics-on-scene, hospital doctors, care homes, HMP services and others. Your role will be to discuss and guide clinicians to the most appropriate NHS resources.

You will provide strategic guidance and oversight to the operational teams and any "bottlenecks" in our acute services will be escalated to you for review. The goal is to facilitate the safest possible care for patients, with the limited NHS resources at your disposal. You will also monitor and review the patient queues by identifying those patients that need priority and those that can wait a bit longer. You will ensure that patients are communicated with regarding any changes or delays to their care.

Location- Hybrid or office based

Role requirements: you must be 2 years Post MRCGP and have at least 1 years' experience at Integrated Care.
