The Role

Great care isn't just about clinical treatment – it's about making sure people receive the right support, at the right time, from the right services.

As a Care Co-ordinator at IC24, you'll play a key role in helping patients navigate their healthcare journey, working closely with GPs, clinicians and wider community services to ensure care is joined up, personalised and centred around what matters most to each individual.

What You'll Focus On

  • Coordinating personalised patient care by managing a caseload of patients and developing care and support plans that reflect individual needs, preferences and goals.
  • Supporting patients to navigate health and care services as a central point of contact, helping them access appropriate services, appointments and community support.
  • Empowering people to manage their own health by providing information, guidance and practical support that enables patients to understand their conditions and access self‑management programmes.
  • Working collaboratively across multidisciplinary teams, including GPs, nurses, social prescribing link workers, health and wellbeing coaches and other healthcare professionals.
  • Supporting multidisciplinary working and service delivery by helping coordinate team meetings, supporting palliative care and safeguarding processes, contributing to QOF activities and assisting practice initiatives that improve outcomes.
  • Building strong relationships with patients and carers through compassionate, responsive support.
  • Contributing to continuous improvement of primary care services by identifying opportunities to improve patient pathways, strengthen collaboration and enhance the overall experience.

What Matters in This Role

  • Experience working in health, social care or another patient‑facing support role.
  • Excellent communication and interpersonal skills.
  • Highly organised with the ability to manage a varied workload and prioritise effectively.
  • Experience working collaboratively within multidisciplinary teams.
  • Confidence in using Microsoft Office and learning other software.
  • Ability to build positive relationships with patients, carers and healthcare professionals.
  • Compassionate, proactive and committed to delivering outstanding patient‑centred care.
  • Commitment to continuous learning, teamwork and service improvement.

Part‑time hours available.
