IC24 is seeking a Care Co-ordinator to help patients navigate the healthcare journey, joining GPs, clinicians and community services to provide joined-up, person-centred care.
This role focuses on coordinating care plans, supporting access to services and empowering patients in self-management.
Based at Herstmonceux Integrative Health Centre, part-time hours available.
