IC24 is recruiting an Advanced Clinical Practitioner for our Home Visiting Service. You will assess patients in their homes, prescribe where appropriate and coordinate care with primary and community teams to ensure safe, timely treatment.
You will work autonomously with advanced clinical skills, maintaining high clinical standards, while escalating to GP or Clinical Consultant when needed. This role requires flexibility, safeguarding and compassionate patient care.
