Job Responsibilities:
- Carry out trusted assessments for adults who are medically fit for discharge from hospital to care home settings.
- Work closely with the Hospital Integrated Discharge Team, multidisciplinary teams, and care home providers to support timely and safe discharges.
- Act on behalf of care homes to assess suitability for admission and manage changes in care needs.
- Reduce delayed transfers of care by resolving barriers to discharge through effective problem-solving.
- Ensure discharge documentation, care plans, and medication are complete prior to transfer.
- Support and promote the effective use of the Red Bag Scheme.
- Liaise with hospital staff, care homes, district nurses, and specialist teams to coordinate discharge arrangements.
- Represent the views and needs of older people, ensuring mental capacity and best interests are considered.
- Provide clear, sensitive, and accurate communication regarding patient needs and discharge plans.
- Monitor discharge quality and report issues to relevant partners, including health and social care teams.
- Maintain accurate records and provide data for performance monitoring and reporting.
- Contribute to service development, pilot projects, and new ways of working within trusted assessment models.
- Support service improvement activities, including training, safeguarding enquiries, and performance initiatives.
- Plan and organise work to ensure effective delivery of discharge support services.
Person Specification:
Must-Have Requirements:
- Professional qualification in Nursing.
- Evidence of post-qualification experience and ongoing continuing professional development.
- Experience working with older people and adults with long-term conditions in hospital and community settings.
- Previous experience of hospital discharge planning or integrated discharge teams.
- Strong knowledge of adult safeguarding, the Mental Capacity Act, and relevant social care legislation.
- Specialist knowledge of the NHS Continuing Healthcare and Funded Nursing Care framework.
- Ability to assess complex needs and recommend appropriate discharge options.
- Excellent communication and interpersonal skills, including working within multidisciplinary teams.
- Ability to manage workloads, prioritise tasks, and meet service deadlines.
- Strong report writing and record-keeping skills.
- Confident use of IT systems for case recording and data reporting.
- Ability to travel within and outside the local area.
- Willingness to work flexibly, including occasional work outside normal hours.
Nice-to-Have Requirements:
- Management or leadership qualification.
- Best Interests Assessor qualification.
- Experience supervising or supporting staff.
- Knowledge of reablement services and programmes.
- Experience contributing to service development or pilot projects.
