The predicted salary is between 38616 - 46141 £ per year.
This is a fixed‑term/secondment vacancy until 30th May 2027 to cover maternity leave. The role is part‑time, 30 hours per week, across a 7‑day service operating 08:00‑18:00.
As a Band 6 SPQ Community Nurse you will provide high‑quality nursing care to patients with complex health needs in the community. Your duties will include:
- Assessment, planning, implementation and evaluation of individual care plans
- Providing nursing interventions and coordinating care within the patient's home
- Managing patients with chronic, palliative and end‑of‑life needs, and performing wound care, catheter care and medication management
- Prioritising visits based on clinical needs
- Coordinating care across multiple agencies and services, acting as a role‑model and clinical resource for Band 5 nurses, Assistant Practitioners and Health Care Support Workers
- Supervising, mentoring and teaching staff; supporting staff development and contributing to service improvement initiatives
- Supporting juniors, acting as a team leader and working closely with the wider multidisciplinary team and third‑sector partners
Eligibility: Applicants must hold a Band 6 SPQ qualification and be a qualified Community Nurse. Desirable is the ability to speak Welsh; English and/or Welsh speakers are equally welcome. Non‑UK or Republic of Ireland nationals need sponsorship to work in the UK.
