The Rehabilitation Health & Care Support Worker delivers delegated care/tasks across a range of disciplines as part of a team-based approach to delivering holistic patient-centred care. Ystwyth ward specialises in Stroke and frailty rehabilitation.

A key aim of the role will be to support the patient to achieve optimum independence by assisting in the provision and monitoring of rehabilitation services; implementing an agreed individualised plan of care as determined by the team.

The post holder is expected to always treat individuals with dignity and respect, recognising their wishes and beliefs, empowering the patient to have choice and control of their care. The post-holder will practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies. The post-holder will be expected to develop within the role as required within the boundaries of the job description and agreed competencies. The post holder must communicate effectively at all times and in conjunction with other team members contributing to and maintaining the team/ward philosophy.

Main duties of the job:

  • Support patients to transition from emergency or acute care to home.
  • Work closely with therapists and nurses to enable people to reach their individual goals and optimum functional baseline.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcomed to apply.

Person Specification:

  • Numeracy and literacy to at least GCSE level or equivalent.
  • Relevant NVQ / Diploma at level 3 or equivalent experience and prepared to work towards achieving this level of qualification within two years of appointment.
  • Record of professional development.
  • Knowledge of rehabilitation approaches.
  • Recent experience of working within a relevant health, social care, third or independent sector service environment.
  • Recognise the importance of the Fundamentals of Care to the patient/client & of enabling patient/client to optimise independence.
  • Experience of rehabilitation.
  • Experience of working as a health & care support worker.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£25,313 to £26,999 a year per annum (pro rata if part time).
