An exciting opportunity has arisen for Band 2 Health Care Support Workers to join the team at South Pembrokeshire Hospital, Sunderland Ward. This is a 40 bedded unit focusing on slow rehabilitation/Frailty Ward. The service is consultant led. Admission is accepted from the community and Withybush General Hospital. The team works alongside community, primary care and the acute site to develop an integrated rehabilitation approach to promoting patient independence and best outcomes for discharge.
Key Responsibilities
- Provide health care support for elderly patients in a slow rehabilitation/frailty setting.
- Assist with daily living activities, mobility and recovery plans.
- Work within an integrated rehabilitation team to promote patient independence and discharge planning.
Qualifications
- Some experience working with the elderly and understanding principles of rehabilitation nursing.
- Full time or part-time hours available.
- Desirable: Ability to speak Welsh; English speakers also welcome.
