Hywel Dda University Health Board is seeking two enthusiastic and motivated Pharmacists to join the Primary Care Medicines Optimisation Team. These roles will work within the Carmarthenshire and Pembrokeshire & Ceredigion localities. This is an excellent developmental opportunity, particularly suited to pharmacists wishing to build experience in primary care or broaden their clinical skill set.
As part of the wider primary care team, you will provide targeted support to GP practices and partner organisations. You will work collaboratively and build effective relationships with GPs, nurses, prescribing clerks, care home staff and community pharmacy colleagues.

The post will be based within a primary care office site and will involve travel to GP practices across the health board. You must therefore be able to travel between sites when requested.
Previous experience in primary care is not essential. We particularly welcome applications from pharmacists who are motivated to learn, develop and grow within a supportive primary care setting.
Main duties of the job
As a Primary Care Pharmacist, you will work under the guidance of experienced senior pharmacists and alongside multidisciplinary colleagues to provide prescribing advice to GPs and other healthcare professionals. You will undertake medication reviews with a focus on improving patient safety, optimising medicines use and promoting cost‑effective prescribing.
This role will support the development of autonomy, confidence and clinical decision‑making, while providing structured learning opportunities and exposure to the wider patient pathway across primary care.
Person Specification
Qualifications & Knowledge
- Masters degree in pharmacy (4 years) or equivalent experience or qualifications
- Registered Pharmacist with the General Pharmaceutical Council
- Professional knowledge of medicines management systems and process in an integrated NHS.
- Diploma in Clinical Pharmacy or equivalent experience or qualifications
- Knowledge of specialist area as described in job advert
- Experience of teaching & supervision of pharmacists, technicians, assistants and students
Experience
- Highly developed knowledge and experience of healthcare
- Demonstrate achievement in at least one area of specialist medicines management
- Demonstrate achievement in more than one area of medicines management

- Flexibility to undertake emergency duty including weekends, bank holiday and evening opening
