Nursing Support Worker (Community)

We are looking for a motivated, compassionate and reliable Nursing Support Worker to join our dynamic Community Nursing Teams based in Pembroke Dock, District Nursing Team. You will work as an integral part of a multidisciplinary team delivering high-quality nursing care to people in their own homes, residential facilities and community settings. This is a permanent position working over a 7-day period, including weekends and bank holidays, covering an 8am-6pm service. This is a rewarding role where you can make a real difference to patients and their families by supporting independence, promoting wellbeing and helping people remain safely at home. Due to the rural area public transport is limited. The ability to travel within geographical area to meet the needs of the service is required.

Main duties of the job:

• Provide high-quality, person-centred care under the direction of Registered Nurses.
• Assist with wound care, venepuncture, pressure area care, catheter care and other delegated clinical tasks aligned with your competencies.
• Support patients with activities of daily living, including personal care, nutrition, hydration and mobility.
• Accurately document care in line with NHS Wales and Hywel Dda UHB policies.
• Record observations and care interventions.
• Work collaboratively with colleagues to ensure safe and effective service delivery.
• Travel within the local community to visit patients.

Person Specification:

Qualifications and Knowledge:

• Level 3 qualification in a health care related subject or equivalent experience and can evidence work based competency demonstrating theoretical knowledge, and the ability to undertake clinical care duties relevant for the role which is gained through experience and worked based training/short courses.
• Knowledge of the Code of Conduct for Healthcare Support Workers in Wales and All Wales Delegation Guidelines (HEIW).
• Knowledge of relevant policies and procedures within a healthcare setting for example the principles of Safeguarding adults/children to keep patients within their care safe, and confidentiality for example General Data Protection Regulations (GDPR).

Experience:

• Experience of working within a relevant health or social care environment, providing direct patient care.
• Experience of working with patient groups relevant to area of practice.
• Experience of working within the NHS.

Skills and Attributes:

• Welsh Speaker (Level 1).

£25,313 to £26,999 a year pro rata.
