This Band 6 District Nurse position is available only to staff currently employed by Hywel Dda University Health Board and based in Lampeter, Ceredigion. The role requires previous community experience and the completion of the Specialist Practitioner Qualification in District Nursing (SPQ), or the candidate must be working towards it. Fluency in Welsh is desirable but not mandatory – English or Welsh speaking staff are encouraged to apply.
Main Duties
- Manage or support a delegated caseload within the Community Nursing Team, delivering high‑quality, complex care in patients’ homes.
- Deputise for the Band 7 District Nurse Team Leader, overseeing operational, professional performance, quality, clinical effectiveness, and resource/budget management of the team.
- Maintain effective communication among staff, patients/carers, and external agencies, ensuring appropriate delegation, risk management, and care coordination.
- Provide care within the home or clinic environment, liaising with patients, families/carers, and other health and social care organisations.
- Travel between sites efficiently within the 8 am‑8 pm 7‑day shift schedule, including weekends and bank holidays.
