PLEASE NOTE THAT WE WILL ONLY ACCEPT APPLICATIONS FROM STAFF CURRENTLY EMPLOYED BY HYWEL DDA UNIVERSITY HEALTH BOARD. **This is a fixed term post until 31st March 2027**
We are seeking an enthusiastic and committed Specialty Doctor in Acute Frailty to join the multidisciplinary team at Withybush General Hospital. This role is centred on the rapid assessment, management and delivery of Comprehensive Geriatric Assessment (CGA) for older people living with frailty, ensuring patients receive the right care, in the right place, at the right time. The post holder will work across the Acute Frailty Unit (AFU) and Emergency Department (ED), supporting early identification of frailty, admission avoidance, timely discharge planning and optimisation of patient outcomes. Working closely with consultants, therapists, nursing staff, pharmacists, social care and community teams, the successful candidate will play a key role in delivering person‑centred care that promotes independence and reduces unnecessary hospital admissions and length of stay. The role offers opportunities to contribute to service development, quality improvement, teaching and professional development within a progressive frailty service.
Main duties of the job
The post holder will provide specialist assessment and management for older adults living with frailty who present with acute illness, deterioration or complex needs. They will undertake and support Comprehensive Geriatric Assessments, develop individualised care plans, and assess and manage common frailty syndromes including falls, delirium, confusion, reduced mobility, infections, dehydration and medication‑related issues. Working across the Acute Frailty Unit and Emergency Department, they will identify patients suitable for same‑day emergency care, admission avoidance or early supported discharge, ensuring patients are directed to the most appropriate care setting. The post holder will coordinate care across multidisciplinary teams, liaise with community and primary care services to support continuity of care, and facilitate safe and timely discharge planning from the earliest opportunity. They will contribute to patient flow management, support service improvement initiatives, participate in clinical governance, audit and quality improvement activities, and provide teaching and supervision to multidisciplinary colleagues and medical trainees as appropriate.
