Band 6 Respiratory Nurse to join the Community Respiratory team in Pembrokeshire. The role is for an experienced registered nurse with a high level of clinical and theoretical knowledge in respiratory nursing. This is a full‑time post, though part‑time work may also be considered. The position is fixed term/secondment until 1 March 2027 due to funding.
Main Duties
- Manage patients with chronic obstructive pulmonary disease (COPD), including post‑exacerbation discharge reviews.
- Undertake capillary blood gas sampling and complete annual home oxygen reviews.
- Support the wider community respiratory CNS team.
- Collaborate closely with multidisciplinary teams to improve patient outcomes and strengthen respiratory nursing capacity.
- Deliver specialist nursing knowledge, care planning and targeted interventions to reduce hospital admissions.
- Work closely with respiratory consultants, multi‑professional respiratory colleagues, primary care and community‑based nursing teams and services.
- Exhibit excellent verbal/written communication and organisational skills.
- Desirable to speak Welsh; English and Welsh speakers are equally welcome.
