Hywel Dda University Health Board (Pembrokeshire) presents an exciting opportunity for an experienced, motivated, and passionate individual. They will join our multi-disciplinary intermediate care team (ICT) as a Clinical Lead Physiotherapist based in Withybush Hospital, Pembrokeshire. This leadership role is part of the team working across the Front of House (ED and Acute Clinical Decisions Unit), prevention of admission; rapid response services and reablement support across the community setting, and as clinical triage being part of the central communication hub - Porth Preseli. The team supports a large variety of patients, presentations, and pathologies often in complex and rapidly changing circumstances. The ICT therapists provide a 7-day service to support prevention of unnecessary admission into the acute hospital.

Working closely with nursing teams, medics, paramedics, ANPs, reablement services, local authority colleagues and the wider team, the aim is to achieve high-quality person-centred care which considers the patient holistically, promoting their independence and wellbeing. The team holds a pivotal role in promoting excellent links between acute, intermediate, community and third sector services.

Main duties of the job

The successful candidate will require extensive post-graduate experience across many clinical areas, which they can apply to the diverse populations supported by the team. They will complete highly specialist assessments and deliver personalised treatment plans with a focus on discharge planning and prevention of admission. The post holder will play a key role in helping patients to stay well at home. The successful candidate will be able to evidence a passion for high-quality care and experience of service improvement. Working in collaboration with other therapy leads and the wider Intermediate Care Team, they will be instrumental in developing the service to meet the needs of the population. Strong leadership, organisational and team management skills are essential, with the need to lead a team in a high-pressure and fast-paced environment.

The Clinical Lead Physiotherapist will support delivery of an in-service training programme and provide operational supervision to the multi-disciplinary team, and clinical supervision to students. They will also support staff supervision and annual reviews. There will be an expectation to work as part of the 7-day intermediate care service, and to hold respiratory on-call competency skills to contribute to the provision of the Respiratory on-call rota.

Job responsibilities

This post will be complemented by the presence of a Clinical Lead Occupational Therapist who works Tuesday - Friday. The post holder will be required to participate in the provision of a flexible 7-day service, based on service need and rota basis. They will have the skills required to participate in the Respiratory On-Call Service. The ability to travel to locations within Pembrokeshire is required to support the community component of the role. There may on occasion be the need to travel to other areas of the Health Board.
