A new challenge? An exciting opportunity? If so, join us in the Fishguard and St David’s District Nursing Team. This is a permanent full-time position, 37.5 hours per week, working as part of a 7‑day rota that includes weekends and bank holidays.

The suitable candidate must hold excellent communication and time‑management skills, have a flexible approach to the needs of the service and work well as part of a team. Community experience is preferred but not essential. The ability to travel between sites in a timely manner is essential.

Main Duties of the Job
  • Provide and be accountable for the provision of a high standard of cost‑effective, safe, individualised nursing care to patients in the community setting through assessment, planning, implementation and evaluation of patient care, based on evidence‑based practice in line with legal requirements, health board policies and the NMC Code.
  • Provide care to adult patients in their own home, residential or nursing care home, clinic setting or holiday accommodation with any medical or nursing condition; hold delegated responsibility for assessment, planning, implementation and evaluation of nursing care packages and contribute to the application process for continuing health care funding.
  • Deputise for the District Nurse in their absence and support the efficient running of the caseload.
  • Contribute to the induction and training of new staff members, student nurses, pre‑ and post‑basic students.
  • Be flexible, adaptable and able to contribute to the performance of the community nursing team while developing professionally within the role.
  • Work alone with confidence to make decisions without supervision.
  • Work in line with the Health Board Lone Worker policy.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties. We have over 13,000 staff and together we provide primary, community, in‑hospital, mental health and learning disabilities services. We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest. Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire. Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings. 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers. Numerous mental health and learning disabilities services.
