Overview
An exciting opportunity is available with the Diabetes Team at Prince Philip Hospital. This is a Fixed Term / Secondment vacancy until 31/3/2026. If you are a Hywel Dda University Health Board employee applying for this post as a secondment, approval from your current manager is required before application. The post holder will work collaboratively with patients, patients’ families, the public and professional staff across health and social care services.
The post holder will lead, develop and deliver a high standard of diabetes services to promote health and enable self-management of diabetes, in consultation with the multidisciplinary team (MDT).
We invite you to apply for the Diabetes Specialist Nurse role. Extensive post-registration experience in diabetes care is essential. The post requires expert and advanced knowledge in managing Type 1 and Type 2 diabetes, gestational diabetes, pregnancy in pre-existing Type 1/Type 2 diabetes, insulin pump therapy and nurse-led clinics. A postgraduate qualification in diabetes management is required; a prescribing qualification is desirable.
Main responsibilities
- Work to the level of Senior Specialist Nurse as outlined by the career and competency framework for diabetes specialist nurse (TREND).
- Demonstrate a high level of clinical nursing skills and keep pace with advances in pharmaceutical, technological and educational treatment for diabetes in both adults and young people.
- Undertake patient assessment, planning and implementation of care.
- Enable people with diabetes to self-manage their condition within their capacity and consent; provide structured education in line with NICE guidance.
- Improve diabetes management by optimizing investigations, interpreting results, and communicating them to patients.
- Monitor diabetes indicators, anticipate decline, and proactively manage patient wellbeing to maintain independence.
- Ensure a safe environment for patients, visitors and staff.
- Understand and apply Health Board policies and guidelines.
- Work with individuals with complex health needs and provide support to junior staff within your locality.
- Collaborate with other specialties and healthcare professionals to improve the diabetes journey.
- Welsh language proficiency is desirable; applications from English and Welsh speakers are welcome.
