We are excited to offer an opportunity for two experienced and motivated rotational Pharmacists to join our dynamic Pharmacy team at Glangwili General Hospital. You will be working across a variety of general medical and surgical wards, within the Emergency Department and the Women and Children's wards, delivering a proactive, patient‑centred clinical pharmacy service.
You will be key members of the multidisciplinary team, contributing to safe, effective and evidence‑based use of medicines to improve patient outcomes. You will deliver a high quality, patient focused clinical pharmacy service by carrying out comprehensive medicines reconciliation, providing expert advice on the safe, effective and appropriate use of medicines, assessing prescriptions for clinical appropriateness and supporting prescribers in complex decision‑making, supplying medication in a timely manner.
You will support the Lead Pharmacist by contributing to the development and implementation of guidelines and protocols, participating in clinical audit, incident reviews and quality improvement projects to enhance patient care.
- Provide expert clinical pharmacy support to CDU and ED.
- Counsel patients to optimise their understanding and adherence to treatment.
- Work closely with multi-disciplinary teams to ensure safe and effective medicines use.
- Actively participate in board and ward rounds to promote safe prescribing practices and facilitate discharge planning.
- Support antimicrobial stewardship and medicines optimisation initiatives.
- Contribute to service development and quality improvement projects.
- Provide education and training by supervising and mentoring junior pharmacists, pharmacy technicians, pharmacy students as well as clinical ward staff.
