An exciting opportunity has arisen for a Band 3 Frailty Support Worker on the Clinical Decisions Unit in Glangwili General Hospital, Carmarthen. The post offers the opportunity to develop your skills and experience in caring for frail patients. Dementia and frailty is a growing challenge within healthcare, and we, as healthcare professionals are at the forefront of providing the dignity, support and care that people deserve. The aim of the role would be to improve the patient experience. You would be expected to promote the patients' independence by focusing on their mobility, nutrition and hydration in order to maintain and maximise their functional ability.

Main duties of the job:
- The post holder should hold an NVQ level 3 in Health or equivalent.
- They will need to demonstrate that they have good communication skills and that they are an excellent team player as well as being able to work alone.
- They will be expected to work alongside both the nursing and the therapies team.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Person Specification:
- Numeracy and literacy to at least GCSE level or equivalent.
- NVQ/Diploma at level 3 in Health or relevant qualification.
- Record of professional development.
Experience:
- Recent clinical experience of working within a relevant healthcare or social care environment.
- Experience of working as part of a team.
- Willingness to attend to further training.
- Recognise the importance of the fundamentals of care.
- Recognise rehabilitation principles and enabling patients to optimise independence.
Language Skills:
Welsh Speaker (Level 1).
