Job Overview
The Critical Care Outreach Lead Nurse is a senior specialist role responsible for leading the Critical Care Outreach service, delivering expert clinical care and operational leadership. The post holder provides advanced assessment, intervention and ongoing support for patients recently discharged from critical care and those who are acutely unwell on wards.
Responsibilities
- Lead the day‑to‑day operation of the Critical Care Outreach team, ensuring safe staffing, effective workload management, and high standards of patient care in line with national guidance and organisational policy.
- Provide advanced assessment, intervention and support to patients recently discharged from critical care and to those who are acutely unwell on wards.
- Make autonomous clinical decisions, perform complex patient assessments, and act as the key escalation point for clinical and operational concerns.
- Drive service improvement through audit, research, and performance monitoring, and lead the development and implementation of evidence‑based clinical guidelines and care pathways.
- Support staff development, wellbeing and professional supervision, and collaborate with multidisciplinary teams to enhance patient outcomes and continuity of care.
- Contribute to strategic service development, governance and the ongoing advancement of critical care and deteriorating patient pathways across the Board.
