A new and exciting development role has arisen within Community Nursing Same Day Urgent Care and the Ceredigion Intermediate Care Multi-Disciplinary Team in Hywel Dda University Health Board. We are looking for an enthusiastic individual to join our Same Day Urgent Care and Outreach Team.
Main duties of the job:
The successful candidate will work alongside the county's Same Day Urgent Care and Outreach Team (comprising Advanced Clinical Practitioners, therapists, Urgent Care Practitioners and health care assistants). The aim is to provide advanced clinical assessment and treatment to patients presenting with a Differentiated/Undifferentiated medical and or intermediate care/nursing need in their own home. They will demonstrate safe, clinical decision-making and expert care, acting within their professional boundaries and qualifications.
The post holder will work within the Same Day Urgent Care service within the Cardigan Integrated Care Centre in Cardigan and support the outreach team based in the integrated centres both in Aberaeron and Cardigan to see, treat, diagnose and discharge where appropriate or attend home visits to include residential and nursing homes, where they will liaise with the intermediate care team, community staff, family and other stakeholders in developing a suitable patient centred management plan for the patient. This will include writing care plans and management plans to be shared with other providers including the Out of Hours (OOH) service.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Person Specification:
Qualifications and Knowledge:
- Essential: Registered Health Care Professional (e.g. HCPC/NMC/GPhC)
- Relevant Master's Degree
- Advanced specialist knowledge of patient pathways
- Advanced theoretical knowledge and practical experience to carry out clinical consultation and/or examination relevant to practice
- Independent Prescribing qualification (if required for role)
- Evidence of maintaining an Advanced Clinical Practice portfolio
- Desirable: Management/leadership qualification or possess equivalent skills and experience
- In receipt or working towards Independent Authorisation of Blood Products (if relevant for role)
Experience:
- Essential: Extensive post registration clinical in advanced practice within specialty area
- Evidence of performing of Advanced Level of Practice
- Proven track record of working collaboratively with multi-disciplinary teams
- Desirable: Welsh Speaker (Level 1)
