Our Community Nursing and Allied Health Care Profession team is changing. With more emphasis on integrated intermediate/community-based care it means there is never a better time to join us. We are looking for an enthusiastic, adaptable and organised person to join our professional team. Could you use your skills and experience to support the delivery of great care? The role is an important one as you will be using your knowledge, experience and expertise to provide support to hard-working frontline teams. The challenges come thick and fast in this role, but you will know that your input is contributing to the excellent experience our patients and service users have every day. The service covers 7 days a week 08:00 - 20:00 including bank holidays.

Main duties of the job

You will be a key member of the Intermediate Care Multi Disciplinary Team which provides a central point for the referral and provision of integrated community services, facilitating safe and timely care provision for our population. You will be expected to be kind and responsive, professional and informative and contribute to the quality of our services by demonstrating the behaviours detailed in our Values. You will receive queries via telephone, email and face to face conversations and will resolve issues using your expertise of pathways and processes. To be successful in this role you will need excellent time management skills, must be able to work on your own initiative, and be able to prioritise your own workload with minimal supervision. The role is diverse, including taking referrals, being part of SDUC/Outreach discussion, liaising and arranging services. You will be working with a number of different professionals in a busy, supportive environment. Some knowledge of health and social care services would be beneficial as you will be dealing with diverse referrals and pathways, gathering information and coordinating responses.

Qualifications and Knowledge

  • Diploma Level (level 4) qualification or equivalent experience and be prepared to work towards diploma.
  • Knowledge of administrative procedures, including IT systems, project management and training knowledge.
  • Awareness of Clinical and Information Governance.

Experience

  • Previous experience in an NHS / Local Authority role.
  • Experience of working under own direction whilst knowing when to seek support or advice.
  • Experience of supporting service improvement.
  • Experience in a customer focussed environment.
  • Evidence of working with IT systems including Word, Access, Excel.

Language Skills

Must be empathetic to and supportive of the aims of the Welsh Language Act. Welsh Speaker (Level 1).
