We have an exciting opportunity for a Band 5 Community Nurse to join our Community Nursing Team in Aberystwyth.
Role Summary: Support the management of a delegated caseload within the Community Nursing Team, providing high‑quality and complex care management to patients in their own homes, under guidance of an experienced District Nurse Team Leader.
Key Responsibilities:
- Deliver high standards of care to patients and families/carers within the home or clinic environment.
- Liaise with staff, patients/carers and other agencies to ensure appropriate delegation of duties and risk management.
- Ensure effective communication between all members of the care package.
- Work an 8 am‑8 pm service over a 7‑day period, including weekends and bank holidays.
- Travel between sites in a timely manner.
- Support professional development and contribute to improving high standards of community practice.
Eligibility & Requirements:
- Current staff members of Hywel Dda University Health Board only.
- Band 5 qualification and experience in community nursing.
- Proficiency in English; the ability to speak Welsh is desirable.
- Willingness to travel between sites.
