Band 5 Community Nurse - Home Visits & 7-Day Support in Aberystwyth

Hywel Dda University Health Board is seeking a Band 5 Community Nurse to join our Community Nursing Team in Aberystwyth. This position supports management of a delegated caseload and provides high-quality care management to patients in their homes.

The ideal candidate will have a Band 5 qualification, current experience in community nursing, and a commitment to delivering high standards of care. Proficiency in English is required, while the ability to speak Welsh is desirable. This role includes an 8 am to 8 pm service over a 7-day period.
