An exciting opportunity has arisen for the role of Patient Flow Coordinator at NHS Practitioner Health. As Patient Flow Coordinator you will ensure that patients move smoothly through our service without delay. As our patient numbers increase and care pathways become more complex, your coordination will be essential to avoid bottlenecks, streamline appointments, and guarantee that each patient receives the right care at the right time., As a Patient Flow Coordinator, you will play a vital role in ensuring that patients receive timely and seamless care. Your responsibilities will include tracking and coordinating patients journeys across our multiple service offerings, ensuring follow-up appointments are scheduled, outcomes are recorded, and discharges are processed efficiently. Working closely with our Patient Services and Data teams, you will address any capacity issues, monitor patient flow, and support clinicians by identifying patients in need of follow-up. Through these efforts, you will help to minimise delays, improve patient experience, and facilitate optimal care pathways.

The ideal candidate will have a background in healthcare coordination, excellent organisational skills, and the ability to work closely with both clinical and non-clinical teams. Familiarity with NHS patient management systems and a commitment to patient-centred care are essential. This is an excellent opportunity to be part of a dynamic team dedicated to ensuring high standards of care and service efficiency for all patients.
