North Lambeth PCN is recruiting 2 WTE PCN Care Coordinators to work with GPs and clinicians at Waterloo Health Centre. You will support patients in managing their care and act as the first point of contact for patients, carers and clinical staff.
Successful candidates will provide administration across practices as well as direct care coordination, focusing on personalised care plans and engaging patients in training and wellbeing activities within the PCN.
