We have an opportunity for an enthusiastic and proactive Care Navigator to join our team on a full‑time basis for 40 hours per week Monday 9:30‑17:30 to Friday.
The Care Navigator is the first point of contact for patients, providing a professional, compassionate, and effective reception and navigation service. The post holder supports patients to access the right care at the right time by sign‑posting, coordinating care, and facilitating the smooth running of the practice.
We are a friendly, supportive, well‑established team based in Wood Green and serve a population of around 11,700 patients. Our clinical team includes 5 GPs, 3 nurses and 1 HCA. With an active PCN we have support from pharmacists, PAs, social prescribers and physiotherapists.
Main duties of the job
The main responsibilities of the role include:
- Understand patient needs using active listening and problem‑solving.
- Direct patients to the appropriate clinician or service.
- Promote self‑care and community services.
Reception & Administrative Duties
- Manage appointments and telephone / online enquiries.
- Process prescriptions, results and messages.
- Maintain accurate clinical records.
- Support clinic flow and reception duties.
- Conduct recalls for different patient groups.
Care Coordination
- Assist with referrals, follow‑ups and complex patient needs.
- Work with GPs, nurses, pharmacists, social prescribers and PCN teams.
Communication & Customer Service
- Provide a professional service.
- Manage challenging conversations calmly.
- Uphold confidentiality.
