Job Description

CHC Nurse Assessor | Band 6Location: Remote (UK-based role)Rate: £26.00 per hourWorking Pattern: Monday to Friday, 9:00am – 5:00pmTeam: Continuing Healthcare (CHC)

What You’ll Be Doing
  • Conduct 8 reviews per week and 3 DSTs per week, all completed remotely in line with the National Framework.

  • Carry out thorough assessments, reviews, and monitoring of care for clients funded via NHS Continuing Healthcare and Funded Nursing Care.

  • Act as Case Manager to facilitate, plan, and evaluate client care in collaboration with families and professionals.

  • Build and maintain strong professional relationships across health, social care, and the independent sector.

  • Provide expert advice and support to colleagues where required.

  • Participate in service development and audits to help drive improvements in patient care.

Requirements & Qualifications

? Registered Nurse with current NMC registration (RGN, RMN, or RNLD)? Demonstrable experience conducting Continuing Healthcare assessments (including DSTs)? Strong knowledge of MDT working, safeguarding, and clinical governance? Excellent communication, negotiation, and conflict resolution skills? Ability to independently manage a complex caseload while working remotely

Desirable:? Educated to degree level (or equivalent specialist experience)? Experience in leadership or advisory roles? Understanding of commissioning arrangements, DOLS, and SEND agenda
