What You’ll Be Doing
As an Agency Specialist Mental Health Practitioner (Band 6), you will support adults (18+) with mental health needs within Primary Care Networks (PCNs), providing early assessment, intervention, and signposting to appropriate services. You will:

Conduct strengths-based assessments and provide evidence-based recommendations.

Work closely with primary care teams, social prescribers, and recovery navigators to support service users.

Facilitate seamless transitions between primary care and secondary mental health services.

Connect clients to community and voluntary sector support.

Provide informal and formal learning opportunities to primary care colleagues to enhance mental health awareness.

Support, supervise, and mentor junior staff where required.
Requirements & Qualifications

Registered mental health professional (RMN, Social Worker, or Therapist) with Band 6 experience.

Frontline experience assessing and managing adults with a range of mental health needs.

Strong knowledge of primary and secondary care services and the wider health and social care system.

Experience in multi-agency working, safeguarding, and risk management.

Commitment to CPD and professional development.

Full compliance including DBS check.

Ability to start ASAP.
