An exciting opportunity has arisen for a Band 6 RMN to join a dynamic Community Mental Health Team (CMHT) in the Tees, Esk, and Wear area. This role is ideal for a skilled and experienced professional who is passionate about providing high-quality care and supporting individuals with mental health challenges., * Acting as a care coordinator within the CMHT, managing a caseload of clients.

Providing comprehensive assessments, care planning, and risk assessments.

Delivering evidence-based interventions to promote recovery and well-being.

Liaising with multidisciplinary teams, families, and external agencies to provide holistic care.

Maintaining accurate and up-to-date clinical records.

Registered Mental Health Nurse (RMN) with a valid NMC registration.

Previous experience in a similar role within a CMHT or mental health setting.
