Schedule: Monday to Friday, 9am-5pm

What You\’ll Be Doing:
Join our client\’s team as a Primary Care Pharmacist and play a key role in supporting medication optimisation and chronic disease management within a GP practice setting. In this role, you will:

perform comprehensive patient assessments

create and implement personalised care plans

deliver evidence-based treatments to optimise patient outcomes

collaborate with multidisciplinary teams to provide holistic care

maintain accurate patient records

Requirements & Qualifications:
To be successful in this role, you will need:

a current and valid GPhC registration

experience working in primary care or GP practice settings

strong communication and interpersonal skills

the ability to work independently and within a multidisciplinary team

a commitment to delivering high-quality patient care

Working Hours & Contract Duration:
This role offers a full-time position requiring availability from Monday to Friday. The contract is ongoing with a strong possibility of extension based on performance and service needs.
