Location: Herefordshire

Duration: Ongoing

Schedule: Monday to Friday, 9am - 5pm with a 30-minute break

Start Date: June/July 2025

Rate of Pay: 30-40 per hour DOE

What You'll Be Doing:

  • Join our client's Primary Care team as a PCN Pharmacist and make a real difference to patient care in the local community.
  • Carry out clinical medication reviews for patients with long-term conditions.
  • Prescribe independently as part of the multidisciplinary team.
  • Provide expert advice on medicines optimisation.
  • Collaborate with GPs, nurses and other clinicians to deliver high-quality care.
  • Keep accurate and detailed patient records.

Requirements & Qualifications:

  • GPhC registration as a Pharmacist.
  • Independent Prescriber qualification (essential).
  • Minimum of 2 years' experience in Primary Care settings.
  • Excellent communication and clinical decision-making skills.
  • A proactive and flexible approach to patient care.

Working Hours & Contract Duration:

This is a full-time, on-site role working Monday to Friday. While this role starts as fully on-site, there is potential for hybrid/remote working in the future based on performance and service needs. The contract is ongoing with scope for extension.
