Contract
- Contract: 20 hrs, Permanent (hours vary across a rolling rota)
Key Responsibilities
- To deliver an accommodation based, housing related support service which will support customers to immediately prevent or resolve homelessness and enable them to develop sufficient, independent living skills in order to move-on, sustain a tenancy and live fulfilling lives.
- To engage with customers on a weekly basis to support individuals to secure, maintain and achieve independent living in the community.
- The co-provision of support plans, risk management plans, crisis intervention and engagement with customers in order to deliver positive outcomes in a service that offers involvement and empowerment for customers.
- To work in partnership with all local relevant statutory and voluntary agencies as required whilst delivering support to customers signposting to services where necessary.
- Working in a timely and coordinated way, being aware of services available and when and how to harness them.
- To support customers to improve and maintain physical and mental health and wellbeing and healthy lifestyles.
- To support customers to establish, maintain and enhance healthy relationships with family and friends and community, through social and meaningful activities, education, training promoting self-identity of risks, strengths, and needs.
- To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers.
- Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives.
Qualifications
- Educated to Level 2 Diploma or equivalent.
- Experience of working in a social care or similar environment.
- Knowledge of Psychologically Informed Environments and Trauma Informed Care Principles and Practice.
- Knowledge of statutory and voluntary support agencies.
- Working knowledge of person-centred, strengths-based approaches to support and care planning.
- Experience of working with others to achieve successful outcomes.
- Excellent communication, interpersonal and problem-solving skills.
- Team working experience, working cooperatively with colleagues in complex, pressurised situations.
- Excellent organisational and time management skills and the ability to effectively manage a caseload.
- Willing to work towards a recognised housing/care/support qualification.
