Key Responsibilities
- To carry out needs and risk assessments, conduct support and care plan reviews and provide care and support to all customers accepted onto the Self Enablement Service.
- To engage with customers on an agreed basis to support individuals to achieve and maintain independent living.
- To provide information, advice and advocacy on welfare rights, housing options, tenancy rights and responsibilities, with a focus on helping customers develop skills and knowledge in these areas.
- To provide information and assistance to customers to enable them to make choices about the development of their social network and participation in their local community, including employment, education, and training opportunities.
- To work with customers to identify support needs which can be met by other agencies, to make referrals to specialist agencies, and develop appropriate casework links with such agencies in relation to customers' support plans.
- To provide care and support in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Registered Manager.
- To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers.
- Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives.
- Educated to Level 2 Diploma or equivalent.
- Experience of working in a social care or similar environment
- Knowledge of the Care Quality Commission and the Key Lines of Enquiry and fundamental standards
- Knowledge of varying Complex Needs and their impact on the ability for a person to access housing or maintain independent living.
- Knowledge of local statutory and voluntary support agencies
- Working knowledge of person-centred, strengths-based approaches to support and care planning.
- Working experience of dealing with complex service users/customers/public
- Experience of working with others to achieve successful outcomes.
- Excellent communication, interpersonal and problem‑solving skills.
- Team working experience, working cooperatively with colleagues in complex, pressurised situations.
- Excellent organisational and time management skills and the ability to effectively manage a caseload.
- Willing to work towards a recognised housing/care/support qualification.
