Key member of the North East Lincs HRS Team.

To provide a programme of person-centred support to customers referred into the HRS Intensive Rough Sleeper Support Service in accordance with contract requirements.

To engage with customers on a weekly basis to support individuals who have a history of rough sleeping or multiple and complex needs and are at risk of rough sleeping referred into the service with high quality structured support with the objective of assisting them to make the transition from a chaotic street lifestyle to sustaining stable accommodation.

The co-provision of support/resettlement plans, risk management plans, crisis intervention and engaging with customers in order to deliver positive outcomes in a service that offers involvement and empowerment for customers.

To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers.

Working in conjunction with the Homelessness Prevention Service and other voluntary and statutory services such as rough sleeper initiative funded partners, mental health, social care and accommodation and support providers.

Hours will be delivered between core office hours and early evening.
