We are currently recruiting a Support Practitioner on a full-time basis for our new Intensive Rough Sleeper initiative, as part of the Housing Related Support service. The Intensive Rough Sleeper Service is responsible for the direct provision of Intensive Rough Sleeper support for those who have experienced rough sleeping or are at risk of rough sleeping with multiple and complex support needs.
Responsibilities
- Providing a programme of person-centred support to customers referred into the HRS Intensive Rough Sleeper Support Service in accordance with contract requirements.
- Engaging with customers on a weekly basis to support individuals who have a history of rough sleeping or multiple and complex needs and are at risk of rough sleeping referred into the service with high quality structured support with the objective of assisting them to make the transition from a chaotic street lifestyle to sustaining stable accommodation.
- Co-provision of support/resettlement plans, risk management plans, crisis intervention and engaging with customers in order to deliver positive outcomes in a service that offers involvement and empowerment for customers.
- Providing a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers, working in conjunction with the Homelessness Prevention Service and other voluntary and statutory services such as rough sleeper initiative funded partners, mental health, social care and accommodation and support providers.
- Hours will be delivered between core office hours and early evening.
Requirements / Qualifications
- NVQ Level 3 or Equivalent education.
- Obtained or is working/willing to work towards a recognised qualification in housing/care/support.
- A sound understanding of the social housing environment including emerging government policy, welfare benefits, local statutory and voluntary support agencies and housing benefit entitlement.
- Experience of working in a social care or similar environment.
- Knowledge of the issues faced by customers that have gone through the Criminal Justice System.
- Knowledge of varying Complex Needs and their impact on the ability for a person to maintain independent living.
- Excellent organisational and time management skills, and the ability to effectively manage a caseload.
- Excellent oral and written communication skills, and the ability to deal with difficult situations with tact and diplomacy.
- Current full driving licence or effective access to public transport.
