Key Responsibilities
- Delivering a package of person-centred housing related care and support to enable adults with a physical or learning disability to live in their own homes as independently as possible.
- The care and support is tailored to each individual's needs and includes advice, guidance and delivery of:
- Maintaining and managing independent accommodation
- Setting up and maintaining essential utilities
- Accessing specialist advice or agencies
- Maximising benefit entitlements
- Money and debt management
- Liaising with other professionals
- Accessing training, voluntary work, education and employment
- Developing independent living skills
- Access to client involvement groups
- Encouraging positive neighbour relationships and involvement in the local community
- Appropriate use of leisure time
- Personal Care
The service is open to anyone over the age of 18 meeting the criteria and living in North East Lincolnshire. The Service is registered with the Care Quality Commission to deliver Personal Care. Shifts vary and may include evening and weekend work.
