An exciting opportunity has arisen for a speciality doctor to work with adults with profound and multiple learning disabilities (PMLD) in Hull. The role will provide specialist input to support individuals, their families and other professionals, both within the CTLD MDT and wider primary/secondary care settings, in order for the individual to be seen and supported holistically as an individual, beyond solely been seen as their health needs as well as supporting the understanding of complex health needs of this client group. The post is at the forefront of supporting the reduction of health inequalities for individuals with PMLD and supporting the wider systems to consider and understand individuals health needs in the development of services and processes. The post will also offer specialist knowledge into considering processes and pathway service developments within the CTLD services for clients with PMLD. There will be close working with the CTLD, the Learning Disability epilepsy service and the acute liaison nurse. As well as linking with primary care and acute hospitals to ensure individuals health needs are met and considered. The role also links closely with paediatricians to plan transitions to adult services.
- Providing medical oversight and consultation to individuals with PMLD and their families/carers.
- Provide care co-ordination alongside other MDT members.
- Addressing health inequalities or vulnerabilities.
- Providing a voice of advocacy and equality in clients accessing healthcare.
- Taking an active lead in developing MDT knowledge to support the health needs of clients with a PMLD within the CTLD team.
- Linking with primary and secondary care for clients open to the PMLD pathway as needed.
- Promote PMLD healthcare needs in appropriate forums.
- Supporting service development for PMLD healthcare where needed.
