Specialist Community Nurse in Malton

This is an excellent opportunity to join our team as a UCR and Intermediate Care Specialist Nurse, covering the Scarborough and Ryedale area, on the beautiful North East coast and surrounding area. We are looking for an experienced registered Nurse, to work with our urgent community response, intermediate care teams providing clinical input in order to facilitate timely discharge from an acute hospital stay or to remain safely at home when facing an unexpected health crisis.

The role includes integrated MDT working targeted at people living with frailty, multi-morbidity and/or complex needs to help them stay independent and healthy for as long as possible. To succeed in this role you will need to be passionate about community healthcare provision, able to tackle key priorities with enthusiasm & have the tenacity to deliver in an ever changing environment.

As a senior Nurse you will have management, coordination and supervision responsibilities for junior staff & students.

• To provide high quality and timely clinical triage for referrals coming into the service, providing advice and guidance to referrers and liaising with internal teams and partner organisations to support patient care delivery.
• To be an integral part of an MDT consisting of Nurses, Physiotherapists, Occupational therapists and Support workers delivering patient centred care to maximise independence, confidence and ability.
• To demonstrate specialist clinical skills in practice for patients in need of UCR or Intermediate Care support.
• Support patients with individualised goal setting, education and care plans to work towards fulfilling their potential in a targeted and realistic way via delivery of evidence based clinical interventions and practice in partnership with patients, relatives, staff, managers, and externally with other agencies.
• To provide support/advice regarding service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies.
• To provide line management, supervision, support, guidance and training to peers, Community Support Practitioners and students.

Qualifications and Knowledge

• Degree or diploma in Adult Nursing.
• NMC registration.
• Full understanding of the practice area and specific role applied for demonstrated on application.
• Prepared to undertake or hold a Specialist Practitioner (District Nursing) qualification, within a defined timescale if not already achieved.
• Evidence of CPD or experiential learning at an advanced level e.g. Postgraduate/Masters level. Applicable to specialist area of service.

Experience

• Previous community-based experience or band 6 experience transferrable to community setting.
• Evidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care.
• Demonstrable experience of working in community nursing at band 6 level.
• Development of specialist/advanced level of practice skills and able to demonstrate the impact of this on practice change/development.

Skills and Competencies

• Ability to effectively transport self between multiple addresses/venues on a daily basis including transporting equipment & clinical stock required for treatment.
• Confidence to work alone in the Community including assessing and treating patients in their own homes and making decisions about their needs.
• Familiar with using Datix system.
• Working knowledge of Trust policies and procedures to effectively lead and manage others.
• Familiar with using SystmOne.
