We are seeking to appoint a substantive Specialist within the Community, Frailty & Therapy Care Group to join our established team of Consultants on the south bank. This newly created role will focus on managing frailty within urgent and emergency care across our UEC footprint, as well as providing clinical cover for the acute frailty ward at SGH.
We are looking for a passionate and dedicated Specialist to join our multidisciplinary team in delivering exceptional care for older adults within the North East Lincolnshire community. As a key member of the service, you will support patients living at home or in community settings, with a focus on enhancing quality of life, preventing avoidable hospital admissions, and managing complex health needs.
Key Responsibilities- Clinical: Provide expert clinical assessment and decision-making for frail older adults at the "front door" of acute care (e.g., Emergency Department, Same Day Emergency Care, Frailty wards).
- Comprehensive Geriatric Assessment (CGA): Lead or support the delivery of CGAs to ensure rapid and holistic assessment of frailty, medical needs, functional status, cognition, and social factors.
- Multidisciplinary Collaboration: Work closely with nursing staff, allied health professionals, geriatricians, social workers, and community services to enable timely, person-centred care.
- Admission Avoidance: Develop and implement plans to avoid unnecessary hospital admissions, facilitating safe discharge or transfer to appropriate community-based services.
- Pathway Development: Contribute to the development and improvement of frailty pathways, including rapid access clinics, discharge-to-assess models, and integrated care solutions.
- Ability to offer sound clinical opinion on range of acute medical conditions.
- Ability to take full and independent responsibility for clinical care of patients.
- Minimum of 12 years full time postgraduate training (or equivalent gained part time).
- At least 6 years' experience in a relevant specialty.
- Ability to manage and lead medical/surgical teams.
- Ability to lead on smooth and efficient running of the unit.
- Ability to organise, prioritise and manage high intensity workload.
- Experience of supervising junior medical staff.
- Commitment to formal and informal teaching of medical staff, AHPs and medical students.
- Evidence of participation in audit.
- Ability to extrapolate clinical research evidence to care for individual patients.
- Full Registration with the GMC.
- Valid ALS certificate.
- MRCP or equivalent.
- Postgraduate degree: e.g. MD, PhD.
- Postgraduate qualification in Education or significant experience in post graduate training.
