Are you a Nurse or Pharmacist with an Independent Prescriber qualification and a passion for delivering evidence-based treatments in Substance Misuse? We are seeking dedicated professionals to join our team in Cumbria and make a meaningful impact in people’s lives, their families and communities. The ideal candidate will possess advanced clinical skills, a strong knowledge of pharmacology, and the ability to autonomously assess, diagnose, and manage patient care. This role involves collaborating with multidisciplinary teams to deliver exceptional patient outcomes and ensure the highest standards of care.
Main duties of the job
- Clinical assessment with or without Prescribing a range of medications (including controlled drugs) to support recovery in line with national and organisational guidelines and policies.
- Prescribe cost effectively and according to best practice and provide prescribing information and advice to Pharmacists, Shared Care GPs and hospital staff.
- Undertake Service User centred assessments, care planning, monitoring, evaluation and reviews in practice and outreach settings.
- Blood Borne Virus (BBV) management (risk assessment, pre and post-test counselling, testing and immunisation).
- Deliver health promotion and harm reduction interventions in a client-centred manner (e.g. safer sex, contraception, nutrition and healthy living).
- Promote and deliver immunisation against Hepatitis B in line with national guidance.
- Contribute to the maintenance of client clinical database; co-ordinate data entry and retrieval searches and reports to meet internal and external service requirements and ensure data security at all times.
- Liaise with pharmacists regarding supervised dispensing and prescription queries.
- Share care with GPs for Service Users in Primary Care Extended Services (PCES)/Primary Care Recovery Services (PCRS).

Qualifications
- First level registered nurse or registered pharmacist
- RCGP Qualifications Level 1 (and 2 or willingness to work towards)
- Proficient in Microsoft Outlook and Office programmes
Experience/Knowledge
- Experience of entering complex information onto an electronic/clinical database
- Experience in delivering evidence based treatments in Substance Misuse
- Experience of carrying out general health care assessments including BBV, and delivering health promotion/harm reduction interventions, advice and information
- Awareness of the social context of drug misuse and the wider implications of behaviour
- Understanding of complexities of primary health care and the constrictions and opportunities inherent in the primary care sector
