Main duties of the job
We are seeking dedicated professionals like you to join our team in Sheffield and make a meaningful impact in people\’s lives, their families and communities.We are seeking a highly skilled and motivated Independent Prescriber to join our dynamic healthcare team. The ideal candidate will possess advanced clinical skills, a strong knowledge of pharmacology, and the ability to autonomously assess, diagnose, and manage patient care. This role involves collaborating with multidisciplinary teams to deliver exceptional patient outcomes and ensure the highest standards of care.

Job responsibilities
- Clinical assessment with or without Prescribing a range of medications (including controlled drugs) to support recovery in line with national and organisational guidelines and policies.
- Prescribe cost effectively and according to best practice and provide prescribing information and advice to Pharmacists, Shared Care GPs and hospital staff.
- Undertake Service User centred assessments, care planning, monitoring, evaluation and reviews in practice and outreach settings.
- Blood Borne Virus (BBV) management (risk assessment, pre and post-test counselling, testing and immunisation).
- Deliver health promotion and harm reduction interventions in a client-centred manner (e.g. safer sex, contraception, nutrition and healthy living).
- Promote and deliver immunisation against Hepatitis B in line with national guidance.
- Contribute to the maintenance of client clinical database; co-ordinate data entry and retrieval searches and reports to meet the internal and external service requirements as appropriate and ensuring data security at all times.
- Liaise with pharmacists regarding supervised dispensing and prescription queries.
- Share care with GPs for Service Users in Primary Care Extended Services (PCES)/Primary Care Recovery Services (PCRS).
- Deliver clinical interventions across hubs in different locations and deliver outreach as required.
