Treatment of all patients Perform telephone consultations, telephone triage and online consultations as needed Support patients to adopt health promotion strategies that promote patients to live healthy and apply principles of self-care Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Perform home visits as agreed Work with other health care professionals to diagnose, monitor, manage and treat long term conditions, including non-drug based treatment methods using a management plan and in line with national, local policies and practice needs. 7. Conform to practice procedures for QOF, prescribing incentive schemes, PBC and other enhanced services and directives as stipulated by the National Bodies/ CCG or Tudor Surgery8. Complete referral letters as necessary for patients seen 8.

Ensure data is entered in an efficient and accurate manner on to the practice computer system 13.Promote confidentiality (in line with Caldicott principles) within the practice 14. Deal with non-NHS correspondence (such as life insurance, holiday cancellations and work related letters). 15. Deal with pathology results communications from Out of Hours Service
