Overview
To work as part of the Making Every Adult Matter team (MEAM) within the Changing Futures service. Establishing a relationship with those who have been identified as having multiple unmet needs, leading on the high complex cases, co-ordinating and providing support to those individuals.
Responsibilities
- Work in partnership with a range of agencies for people who are facing multiple exclusions or are unable to access services. Partners include mental health services; probation; drug and alcohol services; commissioned hostels; housing providers and primary health care services.
 - Provide support and guidance to MEAM Officers to ensure effective work with people facing multiple unmet needs.
 - Lead on organising and chairing professional meetings when required.
 - Promote/provide a multi-agency approach to customers who are facing multiple unmet needs, experiencing a combination of problems such as rough sleeping, substance misuse, mental/physical health and repeat contact with Criminal justice.
