We are seeking committed and experienced individuals to join our established Out of Hours Senior Care Organiser Team. This is an exciting opportunity to become part of a highly valued service that provides essential support to vulnerable adults across Hull outside normal office hours.
The Out of Hours Team operates seven days a week and delivers city-wide coverage through a rota system. Shifts for these posts are provided between 3:00pm and 11:00pm, 11:00pm and 7am Monday to Sunday. In addition, a daytime shift operates from 7:00am to 3:00pm on weekends and Bank Holidays. The service plays a critical role in responding to safeguarding concerns, welfare issues and emergencies during evenings, nights, weekends and Bank Holidays when many statutory and regulated services are unavailable.
The team acts as a vital safety net for vulnerable adults and is an integral part of the city's emergency response arrangements.
Working closely with agencies such as Humberside Police, Health Services, Hull Royal Infirmary and Humberside Fire and Rescue Service, the team provides expert advice, timely decision-making and responsive interventions to manage risk and protect vulnerable adults during times of crisis.
In this role, you will work with people who are known to Adult Social Care, as well as those referred into the Out of Hours Service when a change in circumstances has affected their safety, wellbeing or independence. You will support individuals through short-term periods of crisis, helping them remain safely in their own homes wherever possible, reducing risks and promoting positive outcomes. You will work collaboratively with hospital services, Active Recovery, locality teams, safeguarding teams and other multi-disciplinary professionals to ensure individuals receive flexible, person-centred support that enables them to live safe and independent lives.
As part of an experienced and supportive team, you will be expected to work autonomously, exercising sound professional judgement and seeking guidance and supervision where appropriate. ROLE AND PURPOSE
The Adult Social Care vision in Hull is of ‘a life not a service’. To be responsible for the delivery of a flexible support service, the post holder will provide diverse; practical, emotional and safeguarding support to people who have an acute or critical need. The service supports the adult social care function outside normal office hours including weekends, evenings and overnight.
To undertake assessments of need to ensure people have access to advice, information and where necessary short term interventions which minimises the need for hospital or residential admissions. The post holder will actively engage in the promotion of a personalised service which focuses on the hopes and aspirations of the person to live well and remain living independently.
This post will work closely with the acute pathway, discharge hub and A&E liaison to support early discharge. They will also, wherever possible, to support people to avoid admission into hospital.
Assessment processes will actively support people to utilise universal services, community resources and networks as far as possible including maximising the use of Telecare equipment and Occupational Therapy. Appropriately promoting the use of Connect to Support and using it to provide people and / or informal cares with the information and advice they need. As far as possible connect people to family, friends and community networks, peer support groups and voluntary and community organisations, and enable access to universal services if this is relevant.
The post holder will work with people who are known to adult social care, and who have been referred into the out of hours service. This may be when a change in circumstances has resulted in risks to a person’s independence or safety. The post holder will support people through such a short-term period of crisis to enable them to remain living independently or to minimise risks.
There is an expectation that the post holder, following training, will undertake the role as an OT Trusted Assessor. This role will contribute to the assessment, identification and resourcing of low level OT equipment where this could support an individual to live independently or as part of a wider package of care.
The post holder will work collaboratively with the Hospital, Active Recovery, other multi-disciplinary Out of Hours Teams, locality teams and Safeguarding Teams to provide flexible and responsive support to people to live safe and independent lives.
The post holder will operate within the legislative context of the Care Act 2014, Mental Capacity Act 2005, suitably address Deprivation of Liberty Safeguards, Making Safeguarding Personal, and understand and use relevant articles of the Human Rights Act 1998..
There is an expectation that the post holder will practice in an autonomous manner seeking guidance and supervision accordingly to ensure core values that underpin the Council’s conduct at work are demonstrated and positive practice is promoted.
To ensure that all service/support provisions for Vulnerable Adults 18+, older people, those with learning disabilities and physical / sensory disabilities and Mental Health is being provided in accordance with statutory, legislative and registration requirements.
To liaise and consult with other to ensure that support is appropriately focused and delivered to achieve outcomes which promote independence and that care and support is personalised to the individual.
PRINCIPAL ACCOUNTABILITIES:
1. To protect the safety and welfare of vulnerable adults, families and their carers, within “Making Safeguarding Personal” principles. Recognise risk indicators of different forms of abuse and neglect. Demonstrate an outcome focused, person centred approach to safeguarding practice.
2. Customer Focus –
To prioritise and undertake assessment to establish eligibility for service of people referred for Social Care support out of hours, signposting to other appropriate Agencies, Health and Council’s teams as necessary.
To provide support to maximise people’s skills, competence and confidence to enable them to live independently. To ensure all staff work within a strengths based approach, which places the person at the centre.
Supports people to exercise choice and self-determination with due regard to legislation and local policies e.g. Positive risk policy.
To work flexibly with other managers to ensure consistency across the whole service in person centred care to achieve individual outcomes promoting independence and safeguarding vulnerable people.
To investigate and resolve first level complaints.
Assist the team managers in ensuring high standards of practice and equality of access to the service and in the liaison and consultation with people, carers and staff to ensure continuous improvement of the service.
To be skilled working with a range of people with differing needs in a range of settings.
3. Performance Management –
To support and monitor the operation of services and ensure that resources are effectively and efficiently deployed acting as the lead decision maker and deputising for managers of Social Care services.
To attend meetings, reviews as required and deputise at meetings Out of Hours for the Operations Managers.
To ensure that activity and performance are monitored and feedback to staff to enable recognition of strengths and areas for development.
To assist the operations manager in the planning of training and development needs of the staff teams and establish and review support programmes for staff to maximise their potential and undertake regular observations of staff to ensure key values are communicated consistently along with identified standards of practice.
Support staff development and using policies and procedures ensure that staff and team performance is maintained.
4. To ensure a consistent social work support function this will align with See and Solve, Active Recovery, Locality teams, Hospital Locality team and Brokerage.
5. To provide leadership, guidance and support out of hours to adults social care teams.
6. Support hospital system with regard to surge and escalation and mobilise resources to meet priorities.
7. To provide support and social care interventions for people in crisis ensuring that packages are closely monitored and reviewed.
8. Statutory Obligations –
To carry out duties in line with Council policy and ensure all new corporate and regulatory requirements are implemented.
