Ensure least restrictive practice is fully supported and embedded. To lead, coordinate and supervise the team in ensuring that what is important to the people is achieved through person centred practice. To support, assist and deputise for the Deputy Care Manager in maintaining efficient and effective day to day operation of the home. In the absence of the Deputy Manager and Registered Manager lead the running of the Residential Unit. The postholder will be expected to contribute to the continuous improvement, coproduction and development of the service to ensure it develops in-line with the needs and aspirations of the customers the people it supports, their families and Hull City Council the strategic vision and goals of Adult Social Care.

Principal Accountabilities

  • To promote and safeguard the welfare of children, young people and/or vulnerable adults.
  • Statutory Obligations - Undertake duties and responsibilities in accordance with the national minimum standards of care issued by relevant bodies e.g. Care Quality Commission (CQC).
  • To provide direct support, guidance, supervision and care which meets the persons assessed needs in accordance with their agreed support plan.
  • Develop and implement risk assessments relating to individuals care and support taking into account the physical and emotional needs of the people we support.
  • Prepare moving and handling risk assessments for individual people and direct staff on safe methods of transfer that comply with current legislation and preserves the dignity of people who use the service.
  • Liaising with the person, their families and carers and other professionals.
  • Taking the lead on standard transitions into the service, referrals for Transport and ensuring the correct person-centred information is included on the care and support plans.
  • Oversee and monitor the completion of care and support plans, including the completion of monthly summaries and consulting with others to make the necessary changes to individual care and support plans.
  • To direct, support and lead Care Workers in the provision of residential services for people with a range of disabling conditions, ensuring that the physical and emotional needs of the people the service supports are met, whilst always promoting person-centred approaches, dignity, privacy and personal choice.
  • Tasks include leading, directing, supporting with and undertaking personal care including continence care, washing, dressing, oral hygiene, assisting with eating and drinking, PEG feeding, postural management, catheter and stoma care, and support and direction with following prescribed diets to ensure safe intake of nutrition and hydration.
  • Ensuring safe administration of medication through providing supervision and direction to staff undertaking medication duties through observations, spot checks, MRC audits and training.
  • To ensure that medication is administered as prescribed, by the medical practitioner and as per risk assessment/health action plan and universal medication policy, through various routes i.e. oral, topical and peg and appropriate records are accurately maintained by Residential Care Workers.
  • Takes the lead in ordering, maintenance and safe disposal of medication stock (including creams and medical appliances).
  • To lead in the support of people with behaviours of concern in line with the persons care and support plans, following all relevant legislation, physical intervention training, good practice guidance and least restrictive principles.
  • Take the lead in and hold staff briefings, ensuring the fair delegation of tasks and responsibilities and the handover of service critical and relevant health and safety information is completed daily.
  • To be responsible for allocating, organising and prioritising work and supporting Activity Organisers to develop programmes of activity, including identifying and sourcing internal / external activities which meet the needs of the people who attend and the services.
  • Making decisions and problem-solving day to day issues to ensure smooth running of the service, such as changing activity rotas, staff rotas and staff deployments due to sickness or other absence.
  • Making amendments to individual services as necessary and deciding whether escalation is required.
  • To set up and maintain effective communication between a range of health and social care colleagues, carers and families, making referrals when appropriate to other agencies or council departments and maintaining effective communication with the people we support, their carers and their families.
  • Attend multidisciplinary meetings with stakeholders for the purpose of best interest outcomes for the people we provide care and support to.
  • Write and co-ordinate reports / information for individual reviews including liaison with other professionals / family, friends and carers.
  • To follow up and track all actions are taken and take further action if necessary.
  • As a Keyworker, facilitate person centred reviews to promote the individuals agreed outcomes.
  • Work closely with the people we support, their families and other professionals to review plans on a regular basis ensure consistency and to maximise the effectiveness of the provision.
  • To contribute to and the formation, implementation and review of agreed behaviour support plans and risk assessments under the supervision of senior staff and in collaboration with the person, their families, carers and other professionals.
  • To lead on the planning and implementation of inclusive and meaningful person centred programmes of activity, outings and opportunities for cultural enrichment for people with a range of different disabilities, which promote their mental, physical, social and emotional wellbeing.
  • Lead on arranging if required, an escort for medical appointments, hospital admissions, social outings working within the relevant risk assessments.
  • Organise and assist the people we support to control their day to day finances, taking into account financial regulations, the mental capacity act and undertaking regular financial audits and best interest decision making in regard to of peoples monies.
  • Complete formal supervisions of Residential Care Workers, Activity Organisers and Domestics.
  • Manage and lead the staff group with responsibility for allocating, organising and prioritising of tasks relevant to the day to day care and support of people using the service and efficient running of the home.
  • This includes sickness absence management, rota management, induction.
  • Undertake competency checks of staff and report any concerns to the Registered Manager/Deputy Manager.
  • To contribute to the recruitment, selection, probation and induction of new staff, and induction and management of volunteers within the service.
  • Assisting / demonstrating moving and handling techniques, observations and competency checks to ensure compliance and identify needs for further training.
  • To be responsible for overseeing the day to day administration of the unit including reporting of repairs and health and safety concerns, statistical returns, undertaking and communicating risk assessments and support senior staff to conduct investigations such as missed medication, medication recording errors, medication administering errors, code of conduct and Health & Safety investigations in line with Hull City Council policy.
  • Support staff through PGPR and training and those who are undertaking apprenticeships and diplomas, encouraging staff development through championing a positive learning environment to maintain a well-trained workforce.
  • Work collaboratively with colleagues, managers and other health and social care professionals in line with Hull City Councils Competency Framework to support the city plan in achieving objectives to improve the lives of people we support.
  • Demonstrate a flexible and adaptable approach, including utilising various communication tools and techniques, using IT equipment and undertaking dynamic risk assessment for unforeseen events.
  • Work cohesively with new colleagues throughout their induction, enabling shadowing to share skills, knowledge and experience to ensure the best outcomes for the people we support and a supportive working environment.
